It's easier to discuss Hodgkin’s lymphoma therapy evolution by history. 40 years ago, 30 years ago, huge progress was made for cure of patients with Hodgkin's lymphoma. But the price of treatment success was toxicity. Many of Hodgkin's lymphoma patients with intensive combined chemo-radiotherapy regimens had cured Hodgkin's lymphoma, but patients had recurrences of Hodgkin's lymphoma. Many patients had significant toxicity and late toxicity after treatment. They had second malignancies, probably due to chemotherapy regimens and radiotherapy.
Some international groups made significant progress in treatment of Hodgkin's lymphoma. I would like to mention specifically the German Hodgkin's clinical trial group. Hodgkin's lymphoma treatment improved with many protocols which followed each other by reducing toxicity while maintaining or even increasing the anticancer effect.
For early stage Hodgkin's lymphoma, the question was radiotherapy or chemotherapy. Sometimes you have to combine chemotherapy and radiotherapy for Hodgkin's lymphoma at the least possible dose.
For advanced stages of Hodgkin's disease, significant progress was made due to better chemotherapy medications and better risk-adapted indication of radiotherapy of Hodgkin's lymphoma. In early stages of Hodgkin's lymphoma, you give the least toxic medications—one medication if possible—or the least toxic radiotherapy alone with the smallest volume and the smallest dose. This is still considered to be safe to treat Hodgkin's lymphoma.
Significant progress in Hodgkin's lymphoma treatment was made with diagnostic imaging, specifically with PET CT imaging. By understanding the metabolic changes of a Hodgkin's lymphoma tumor, you can risk-adapt your treatment. You can see early changes in the metabolic tumor imaging.
For these questions, the Hodgkin's lymphoma treatment guidelines are important. Hodgkin's lymphoma treatment guidelines are a result of the international clinical trial protocols, which are very often published in important journals. Dr. Stephan Bodis, MD: This is a very important point. Sometimes you're actively participating within a Hodgkin's lymphoma clinical trial group and use their guidelines—that's the logic. Sometimes you're not actively participating. I think it's nice you can follow one or two guidelines for treatment of Hodgkin's lymphoma. You write in your Hodgkin's lymphoma treatment recommendations that you stick and adhere to those guidelines. Various Hodgkin's lymphoma guidelines very often exist because there is no one worldwide standard.
